If you are a Journalism and Professional Writing Concentrator, you should be turning in your portfolio with your DiG form.
If you are looking to change your major, add a major, or add/drop a minor, there is no form for students. You should contact the chair of the department that offers the major/minor. Department Chairs fill out the change/add/drop major forms.

You need to complete and submit this in the semester prior to when you plan to graduate. Do you want to graduate next semester? You should be doing a DIG right now!
The Student Exception Form is used to get a course approved for or directed to a particular requirement. Exception forms need to be signed by the Department Chair; in most cases, you should work with your Advisor if you need an Exception form. Exception forms are used for a variety of situations, including (among other things): 1) to have one course replace another, 2) to waive a particular requirement, or 3) to direct a course to fulfill a particular requirement.
You can Delete course grades. It's true. The specific details are at the Registrar's site, We recommend that you speak to an advisor before doing a deletion; however, you do not need the signature of an advisor or any faculty in order to delete a course.
f there is a special situation of some kind, you might need an Academic Petition. It is NOT the same as an Exception form. Petitions are generally for issues that need to go beyond/above the department. They are almost always signed by the Dean of FAHSS, and often also need to go to the Provost. In all likelihood, you will need to talk to your advisor about your Petition, and in any case, your Advisor is the first level of signature on the form.
If you want to take a course at another school for any reason, at any point in your UML career, you must fill out the Prior Authorization for off campus Courses form BEFORE taking the course.
